Only what you type into the form on this page: your practice name and postcode, your name, your email address, and your phone number if you choose to give it. When you send the form, we also record which link or ad brought you to this page, taken from the web address you arrived on. No cookies are used for this. We do not collect patient information through this page. When you check your area, your postcode is sent to a public postcode lookup service (postcodes.io) to find your area name; nothing else is sent.
If you click Accept on our cookie banner, we use the Meta Pixel, a tool provided by Meta Platforms Ireland Ltd. It sets cookies and tells Meta that you visited this page, used the area checker, or sent the form, so we can measure and improve our advertising to dental practices. We do not send Meta the details you type into the form. If you click Decline, or make no choice, the Meta Pixel does not load and no advertising cookies are set. Our lawful basis for this is your consent, which you can change at any time using Cookie settings. Meta's own use of the data is covered by its privacy policy.

Legitimate interests: contacting dental practices about a service relevant to their business. You can object at any time and we will stop.
Our team; Web3Forms, which delivers form submissions to our inbox on our behalf; Meta, if you accept cookies (see above); and our website hosting provider. Some data may be stored outside the UK under appropriate safeguards.
Twelve months from your enquiry, or until you ask us to delete it, whichever comes first.
You can ask us for a copy of what we hold, ask us to correct or delete it, or object to us using it. Email admin@dentifyd.com and we will respond within one month. You also have the right to complain to the Information Commissioner's Office.
